An unannounced monitoring inspection was initiated by 2 licensing staff on May 2, 2019 from 10:30 a.m. to 12:30 p.m. A census of 6 residents was reported. A follow-up visit was conducted by the licensing inspector on May 10, 2019 from 8:00 a.m. to 9:00 a.m. to complete the monitoring inspection. Previous violations were reviewed and corrected. The following items were reviewed/observed during the inspection: sample of 4 resident and 3 staff records, facility documentation, tour of the facility, lunch meal/menu/activity schedule, medication observation/physician's orders/Medication Administration Records (MARs).
