Comments:

An allegation of neglect and potential verbal abuse of residents was received in the licensing office. Based on interviews with various staff and families there was not a preponderance of evidence to support the allegation. No records could be found containing documentation of incident reported. The complaint was anonymous and there were no specific residents identified making it more difficult to substantiate the complaint. No contact information was provided by the complainant to afford this inspector an opportunity to secure needed additional information despite relaying that no information regarding the complainant will be provided to the facility. The biggest concern raised by family was the lack of consistency and variety in activities. The facility has another inspection due in July, 2019 and this issue will continue to be monitored.
