22VAC40-73 BUILDING AND GROUNDS

Comments:

An unannounced monitoring inspection was completed on May 23, 2019, from 11:46 a.m. to 2:06 p.m. for Saint Francis Home located in Richmond, Virginia. Upon arrival the licensing representative met with the facility?s administrator to initiate the entrance conference. The facility confirmed the census as one hundred and sixteen (116) residents in care. After the entrance conference, the administrator and the licensing representative toured the facility in order to monitor levels of compliance. Residents were observed in daily activities and staff members were observed providing direct care services. The record review encompassed ten (10) resident records and five (5) staff records. During the monitoring inspection one new violation was cited. Please submit a written plan of correction within the next ten calendar days. Your plan should specify how you will correct each violation , an intended plan for future compliance , a date of correction for each violations , and the job title of the responsible staff for implementing and monitoring preventive measures. Description:	VIOLATION: Based upon the building tour, the facility failed to maintain the interior and exterior of the building. EVIDENCE: During the building tour, the licensing inspector noticed a golf ball size hole in the bathroom ceiling located in room L-4. Also, some of the subway bathroom tiles located around the sink in room L-4 had small holes where a soap holder or paper towel rack was removed.
Plan of Correction:	A maintenance request has been completed requesting that the small hole in the sheetrock be completely repaired and the holes be filled in as appropriate. Education of Environmental and Maintenance staff to be observant of similar concerns in the future. Would anticipate completion by 06/1/2019
