Comments:

Two licensing inspectors conducted a complaint investigation on 05/21/2019 at Commonwealth Senior Living at Radford. The allegations being reviewed were regarding appropriate staffing to resident ratios in the memory care unit of the facility. Based on a review of the evidence, the complaint was determined to be not valid. No violations are being cited as a result of this complaint investigation. An exit interview was held on 05/21/2019 with the in charge person on the evening shift.
