Comments:

The information in this initial inspection report will be reviewed by the licensing administrator and the facility will be notified by mail regarding their license status. This inspection was conducted on 06/17/19 due to a change of the licensee. There were 83 residents in care. The facility was clean and free from any foul odors. All required postings were in place. All outside health and safety inspections were current as were related drills. Licenses and certifications were reviewed. The activities calendar and lunch menu reflected what the LI observed. Interviews were conducted with residents and staff. June medication administration records and physician's orders were reviewed for a selected number of residents. Criminal history reports were reviewed for all staff hired since the last inspection. A selected portion of resident and staff records were reviewed.
