Comments:

An unannounced inspection of a self-reported incident was conducted on 6/12/2019 from 1:30 p to 2:15 p after a resident eloped from the memory care unit. One violation was cited. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description:	Based on a review of a self-reported incident report the facility failed to provide a security monitoring system that prevents residents from exiting the facility. Evidence: A resident was observed on camera by the concierge sitting on the curb in the community parking lot. The resident exited from the southeast emergency exit door in the memory care neighborhood.
