Comments:

Two licensing representatives conducted an unannounced focused monitoring inspection at Commonwealth Senior Living at Cedar Bluff on 06/18/2019. The inspection started at 10:30 am and concluded at 12:33 pm. The purpose of this inspection was to determine compliance and correction of previously cited violations. A sample of resident files were reviewed along with the previously cited violations to assist in the process of determining correction and compliance. As a result of this inspection one violation is being cited. An exit meeting was held with the facility administrator on 06/18/2019 and at this time opportunity was given to find items that could not be located in the files. Please provide a plan of correction and date to be corrected for the one violation cited and return a signed and dated copy of the violation notice back to your licensing inspector within 10 calendar days (06/28/2019) of receipt. Standard #:	22VAC40-73-320-A
Description:	Based upon review of resident records, the facility failed to obtain all required information on the physical exam performed prior to admission for one resident. EVIDENCE: 1. Resident #1 was admitted the facility on 02/04/2019, his physical was dated 01/15/2019. The question addressing Resident #1's ability to self administer medications was left blank.
Plan of Correction:	Resident #1 History and Physical was reviewed and updated by Physician on 06/18/19. ED/RCD will assure all H&P's are completed accurately and on filed prior to admission. ED/RCD will reconcile all current H&P's for compliance. Date of compliance 7/15/19 ED/RCD will monitor for ongoing compliance. [sic]
