The licensing office recieved a complaint and a facility report in the same incident regarding allegations of inappropraite sexual conduct between a staff member and a resident. The licensing inspector conducted phone interviews with both the Administrator and the Family Services Specialist at the Local Department of Social Services. Documentation was reviewed regarding the allegations in this report. As a result of this complaint investigation there was not a preponderance of evidence to support the allegations; therefore, this complaint is determined to not be valid. Please return this signed document back to the licensing office within 10 calendar days (07/06/2019) of reciept. If you have any questions or concerns, please contact your inspector at 276-608-3514.
