Comments:

On 06/27/2019 the assigned inspector was on site to conduct a focused monitoring inspection to follow up on previously cited violations. During the entrance interview the Administrator was not on site but arrived shortly thereafter and was made aware of the purpose of the inspection. Prior to the arrival of the facility Administrator the inspector met with the D.O.N and discussed the purpose of the inspection. Based on the review of facility records and interviews conducted the facility has corrected previously cited violations and is demonstrating a willingness to maintain substantial compliance.
