Comments:

An unannounced focused inspection was conducted on 7/2/19 by 2 licensing inspectors from approximately 11:15 a.m. - 1:15 p.m. to follow-up on areas of non-compliance. There are currently 4 residents in care at the facility. A sample of 3 resident and 2 staff records, associated with the non-compliance were reviewed. Facility documentation was reviewed. A tour of the interior and exterior of the facility was conducted. The facility has followed through with corrective plan submitted to the department. An exit meeting was held with the administrator.
