Comments:

On 6/17/2019 one inspector conducted an investigation of a self reported incident. 48 residents were in care. Staff people were interviewed, a resident record was reviewed, and other documents were reviewed.

Violations:

Standard #:	22VAC40-73-550-C
Description:	Based on interview and document review, the facility failed to ensure that a resident was treated in a manner consistent with the rights and responsibilities as provided in ? 63.2-1808 of the Code of Virginia and this chapter. EVIDENCE: 1. Documentation sent by the facility to the licensing office shows resident 1 was mechanically restrained by staff 1 and witnessed by another staff member on 6/2/2019. This is inconsistent with ? 63.2-1808 of the -15Code of Virginia, subsection A-15.
Plan of Correction:	The associate who participated in the mechanical restraint was immediately removed from working with any resident in the facility. The associate who observed but did not report the mechanical restraint was removed from working with any resident in teh facility as soon as management had knowledge of her observation of the incident. Training in the area of Abuse, Neglect, Exploitation has been conducted. Training in the area of Mandatory Reporting has been conducted. The Executive Director has met with all staff to train in the area of Resident Care and the responsibility of out accociates to provide respectful care to all of our residents.
