Comments:

Two Representative with the Division of Licensing conducted an unannounced, mandated, renewal inspection on 07/08/2019 from 9:45am to 11:45am. At the point of entrance the facility staff in charge was present and the facility Administrator arrived later to complete the inspection. The facility had 12 residents care. During the Licensing inspection the Licensing Inspector reviewed 6 resident records and 3 staff records, observed the facility physical plant, reviewed the facility medication administration pass, and observed residents engaged with facility supports. Areas of non-compliance are identified on the violation notice. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring preventative measures. Description:	Based on staff record review the facility failed to ensure each direct care worker maintained First Aid certification. Evidence: on 07/08/2019 with a Representative with the Division of Licensing, it was observed that staff #1's First Aid certification expired on 07/07/2019, staff #2's certification expired on 06/10/2019 and staff #3's certification expired on 07/07/2019.

Description:	Based on observation of the facility physical plant the facility failed to ensure residents do not share soap. Evidence: On 07/08/2019 with a Representative with the Division of Licensing and the facility staff in charge, in the first floor rest room multiple bars of unlabeled soap were found on the shower floor and shower ledge.
