Two licensing inspectors conducted a one day focused monitoring visit regarding a self-reported incident on 07/16/2019. The focused monitoring visit began at 10:15 am and concluded at 10:50 am. The licensing inspectors observed the building, a resident room, and interviewed a resident. An exit meeting was held on 07/16/2019 with the administrator of the facility.
