Comments:
Two licensing inspectors conducted a one day non mandated inspection at Greendale Home on 07/16/2019 regarding a complaint that had been received by the licensing office. The complainant reported there was an issue with an infestation of centipedes in the downstairs area of the building. The Inspectors found several insects in the downstairs part of the building, however; this issue is being addressed and an extermination invoice was provided to the inspector by the administrator. As a result of this inspection 2 violations are being cited. Please develop a plan of correction for each cited violation along with a date of correction and return a signed copy back to the licensing office within 10 days (07/27/2019) of receipt. Description: Based on observations made during the tour of the building, the facility failed to ensure all doorways used for ventilation were effectively screened. EVIDENCE: 1. The Licensing inspector observed a door propped open through the first hallway leading to an outside area in the downstairs part of the building. The doorway did not have an effective screen in place.

Plan of Correction: Doorways used for ventilation shall be effectively screened, this is a exit door and should not be propped open, Residents have been educated as to why the door cannot be propped open, Residents and staff will monitor this door to keep closed. Maintenance and Housekeeping will monitor the door daily for compliance. [sic]

Description: Based on the tour of the building, the facility failed to keep all areas of the building clean. EVIDENCE: 1. The Licensing Inspector observed several insects both dead and alive on the floor and in resident rooms throughout the downstairs part of the building.

Plan of Correction: The interior and exterior of all buildings will be maintained in good repair and kept clean, and free of rubbish. Housekeeping will monitor and clean downstairs daily and more when necessary to insure free of rubbish. Residents have been educated to report to staff any sightings of bugs whether dead or alive. Housekeeping and Direct Care Staff will monitor for compliance daily. [sic]
