Comments:
This monitoring visit resulted from an APS (adult protective services) complaint received on 5/23/19 about bed bugs. At 1:12 pm, the inspection commenced and concluded at 3:48 pm. One violation was cited. The investigation confirmed the complaint is valid. Six residents were in care. During the inspection the following was reviewed: resident record review; physical plant observation; communication log review and conducted interviews. After the inspection, the LI and staff reviewed the violation and had an open discussion. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to your licensing inspector within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain the following: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on physical plant observation and staff interview, the facility failed to be free from insect infestations. Evidence: 1. Staff 1 confirmed the facility had bed bugs since January 2019. Staff 1 penned a 5/23/19 memo to all staff indicating the infestation was to the point that no new residents would be admitted until the bed bug problem was under control. 2. On 5/22/19, the communication log documented resident 1 was sent to the emergency room (ER). The ER nurse called the facility reporting four bed bugs were discovered on the resident. 3. On 5/28/19, the communication log indicated bed bugs were killed in the office.

Plan of Correction: The Board of Directors shall be involved in the up keep of the grounds and the facility. We have had terminax to come in and do an entire sweep of the facility. Then they came back to a follow-up in one week. then again a follow-up was done two later. Then will be doing monthly visits for pest control. They also offer free inspections for staff to check there home to make sure none were transported. everyone home was cleared. All residents beds has new mattresses and boxsprings as well as cover protectors for each.
