Comments:
The LI for Runk & Pratt Residential Adult Care of Lynchburg conducted an unannounced renewal study at the facility on 7/18/19 from 8:40am until 3:00pm and noted 67 residents to be in care. Resident and staff records as well as other forms of facility documentation were reviewed and interviews were conducted with residents and staff. A tour of the facility physical plant was conducted and the morning medication pass and mid day meals were observed. Violations:
Standard #: 22VAC40-73-680-I
Description: Based on observations of the facility medication pass and review of resident records, the facility failed to ensure that all required documentation was included on resident medication administration records (MARs). EVIDENCE: 1. The LI noted a physician order for resident 1 for Fluticasone Prop Nasal Spray, 2 sprays into each nostril daily. The LI observed that only 1 spray was administered into each nostril as resident 1 refused the second spray. Staff initials are present for administering both sprays and there is no documentation that only one spray to each nostril was administered.

Plan of Correction: A physician order was obtained for resident 1's Fluticasone Prop nasal spray for 1 spray to each nostril daily. Director of Nursing will in-service all RMA's on medication administration and proper documentation to ensure physicians orders are being followed as prescribed and recorded on MAR's.

Description: Based on observations of the facility physical plant, the facility failed to maintain the interior of the building. EVIDENCE: 1. The metal cover in the hallway outside of room 145 was loose and raised from the floor. 2. Carpet stains were noted in the hallway throughout the safe, secure unit. 3. The over head light in the bathroom for rooms 104 and 105 does not consistently cut on when the light switch is flipped.

Plan of Correction: Administrator had maintenance repair metal cover in the hallway outside room 145 to fit properly to the floor. Administrator has had carpet cleaned to remove stains throughout the safe secure unit. Administrator had maintenance repair light switch in bathroom for room 104 and 105 to work properly.

Standard #: 22VAC40-90-40-C
Description: Based on a review of staff records, the facility failed to ensure that any person required by this chapter to obtain a criminal history record report was not employed at the facility if the report contains convictions of the barrier crimes. EVIDENCE: 1. The record for staff person 3, hired on 6/18/19, has documentation that the employee was convicted of a felony barrier crime in 2013, which would make her ineligible for employment per this standards requirement.

Plan of Correction: Employee was dismissed from employment. Administrator or designee will review all new hires for any barrier crime convictions to insure all new hires are eligible for employment.
