Comments:

This inspection resulted from violations received during May 2019's inspection. One repeat violation was cited. At 11:09 am, the inspection commenced and concluded at 11:24 am. The census was 15 residents. During the inspection, the LI reviewed the following: physical plant observation and medication administration record review. After the inspection, the LI and staff reviewed the violation, discussed possible corrective actions and had an open discussion. Please complete the correction plan and correction dates for each violation cited on the violation notice and return it to your licensing inspector within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain the following: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description:	Based on documentation review and staff interview, the facility failed to ensure all medications were administered in accordance with the physician's or other prescriber's instructions and consistent with the standards of practice outlined in the current registered medication aide curriculum approved by the Virginia Board of Nursing. Evidence: 1. Resident 1 is prescribed Fentanyl patch every 72 hours for pain. At 11:18 am, staff 1 confirmed it was not administered as scheduled on the MAR for the 7/8/19 8am dose. The MAR was blank for the aforementioned dose. The last administration was given on time on 7/5/19. This is the third time this violation was cited during this licensure period.
Plan of Correction:	The provider?s response for the ?plan of correction? was not received as of 7/26/19 and will not appear on this Violation Notice.
