Technical Assistance:

Recommendation was made for new key staff to attend Assisted Living Facility (ALF) Phase II training.

Comments:

An unannounced monitoring focus inspection was conducted on 7/29/19 by 2 licensing inspectors from approximately 11:15 a.m. - 12:15 p.m., to follow-up on areas of non-compliance from violation notice dated 2/19/19. The current total census reported for ALF and the Safe, Secure Unit is 57. A tour of the safe, secure unit was conducted and facility documentation was reviewed.
