Comments:
The Piedmont Licensing Office received an anonymous complaint in regards to Meadow Hills Assisted Living Facility. The complaint alleges that a resident?s oxygen equipment was broken and he was sent to the hospital without any information. The complaint also has that the facility smells of urine, is dirty and there are no licensed staff in the building. The LI for Meadow Hills Assisted Living conducted an unannounced complaint investigation at the facility on 11/13/19 from 1pm until 4pm. A tour of the facility physical plant was conducted and interviews were conducted with residents and staff. Resident records as well as other forms of facility documentation were reviewed. Based on all evidence available for review it was noted that resident 1, admitted to the facility on 7/6/18 from a hospital stay due to acute COPD exacerbation, had a physician order for oxygen 2 litters/min via nasal cannula as needed. Interviews with resident 1?s veteran affairs worker and staff indicated that his oxygen concentrator was broken temporarily but portable oxygen tanks were available for use until the concentrator was fixed, but that resident 1 frequently refused to wear the oxygen when it was needed. On 9/19/18 resident 1 sustained a fall in his room while retrieving his walker. Staff interviews and documentation in his record express that resident 1 was alert, talking and had no complaints of pain from the fall. Blood was noted on resident 1?s left forearm and Local EMS service were called to transport resident to the local hospital for further follow up and admitted for medical reasons. His personal information and medication administration record was sent from the facility with the local EMS. Resident 1?s PAS was aware of the incident within 24 hours as per regulation requirements. The LI conducted a tour of the facility physical plant and noted no foul/urine odors on the day of inspection. Interviews with residents were conducted and no housekeeping concerns were voiced. The LI reviewed the facility employee schedule and noted that RMA and direct care staff who are qualified to meet residents needs were properly scheduled for each shift through the current schedule.
