An unannounced monitoring inspection was conducted by two Licensing Representatives on August 5, 2019 from 1:09 p.m. to 4:08 p.m. regarding a self-reported incident. The administrator was present throughout the inspection. There were 77 residents in care. Resident records and private duty personnel records were reviewed, interviews and facility tour were conducted. No violations were cited. The following was discussed: private duty personnel documentation, weekly menu postings, monitoring wounds, and Individualized Service accuracy and plan updates.
