Comments:
Two licensing inspectors conducted an unannounced non-mandated complaint inspection at Carrington Place of Wytheville on 08/30/2019. The inspection started at 11:20 am and concluded at 12:11 pm. The licensing office received a complaint regarding resident care and related services at Carrington Place of Wytheville on 07/30/2019. The allegations within this complaint were not supported based on evidence gathered during the inspection and therefore the complaint is invalid. There were issues found that were not reported in the compliant and as a result of this inspection 1 violation is being cited. An exit meeting was held with the administrator on 08/30/2019 and at that time the findings of the inspection were reviewed. Please develop a plan of correction and submit the plan along with a signed and dated copy of the violation notice to the licensing office within 10 calendar days (09/14/2019) of receipt. Description: Based on review of resident records and physicians orders, the facility failed to ensure all physician's orders are maintained in the resident's record and organized chronologically.

EVIDENCE:
1. Resident # 1 had a doctors appointment on July 29, 2019 for lab work. The nursing staff was unable to locate the physician's order regarding this appointment. The physician's order was faxed to the facility on the day of the inspection, however; the order was not maintained in the file.

Plan of Correction: 1. Resident # 1 physician order for lab work while at a doctor's appointment on 7/29/19 faxed to the facility and placed in the medical record 8/30/2019.
2. Quality review of current residents completed by the Director of Nursing (DON)/designee to ensure orders received from a physician's office are completed, maintained in the medical record and organized in chronologically order.
3. RMA's/LPN's to be re-educated by the DON/designee to ensure orders received form a physician's office are completed, maintained in the medical record and organized in chronologically order.
4. Quality monitoring to be completed by the DON/designee through random chart reviews to ensure orders received from a physician's office are completed, maintained in the medical record and organized in chronologically order 3 x weekly x 2 weeks, 2 x weekly x 4 weeks then weekly and PRN as indicated. Findings to be reported to QAPI committee monthly and updated as indicated. Quality monitoring schedule modified based. [sic]
