Technical Assistance:

1. Facility needs new agreements and disclosures when change of ownership occurs; 2. Current fire inspection must be completed prior to change of ownership; 3. Alternative times need to be reviewed for third shift fire drills; 4. Change in dementia training hours and time frames needs to be closely monitored; 5. Continue to make adjustments as necessary in policies and procedures. Census 40 11:30am - 6PM

The information contained in this initial inspection report will be reviewed by the licensing administrator. The facility will be notified by mail regarding their licensing status. This unannounced initial inspection was completed as part of the process related to the change in ownership of the facility from Greenfield Senior Living to Commonwealth Senior Living. The following areas were identified as requiring attention both now by the current administration and continued at the time of change in ownership: 22 VAC 40-73-450 Individualized service plans(incomplete/not available) 22 VAC 40-73-640 Following medication management plan/scope of practice 22 VAC 40-73-650 Physician's orders 22 VAC 40-73-660 Medication Storage/scheduled counts 22 VAC 40-73-680 Medication administration - parameters for glucose checks, blood pressure checks, oxygen status checks and behavioral interventions prior to administration of medication given as needed/PRN 22 VAC 40-73-1110 Appropriateness of placement documents The facility was clean and odor free. Residents raised no concerns regarding their care. They were aware of the change in ownership and letters had been sent as required by the standards. Activities and meals were provided as posted. Drills were current and the facility is waiting for their fire inspection. It is to occur before the change in ownership happens. A new health inspection is due in November.
