Technical Assistance:

If you have not already done so you must submit a completed Renewal Application prior to the expiration of your current license. The application can be obtained from our web site. While the current Health Inspection remains in compliance it will expire prior to the issuance of the new license. Please email or fax a copy of the new Health Inspection to be obtained.

Comments:

An unannounced renewal inspection was conducted on 9/9/19. At the time of entrance eight residents were in care. The sample size consisted of four resident records, and three staff records including a review of criminal background checks for new hires since the last mandated inspections. Additionally, individual interviews were conducted. Residents were observed eating breakfast engaging in activities including including , morning exercise and a music time. Medication administration was observed, and no violations were issued today. Exit interview held with Administrator.
