Comments:

Two licensing inspectors along with a worker from Tazewell County Department of Social Services conducted a complaint investigation at Dogwood Crossing on 09/09/2019. The inspection started at 10:35 am and concluded at 11:21 am. During the inspection staff interviews were conducted, documentation and the resident record was reviewed, additional information was gathered and the resident was observed. Based on interviews and documentation review there was not a preponderance of evidence to support the allegations reported. The complaint is determined to not be valid. An exit meeting was held with an in-charge staff member as the administrator was out of the office. At that time opportunity was given to find items not available in the resident's record. .
