A initial inspection was completed on 9/24/2019. Guidelines for a facility under new ownership was reviewed and signed.

Comments:

Date of Inspection: September 24, 2019; 9am to 12:30pm Type of Inspection: Initial inspection Census 33 The process for completing a change in ownership, CHOW, was reviewed today. The facility has a planned date of October 1, 2019 for the CHOW. A recommendation will be made to have the conditional license effective on that date. Please contact me if the date needs to be changed. The conditional license will not be received in the facility prior to October 21, 2019.
