An unannounced monitoring inspection was conducted at the facility to follow-up on risk violations cited at 7/10/2019 inspection. A daily sign-in log sheet and pre-admission screening form has been put in place, service plans have been updated, and staffing changes have been made. The facility was found in compliance. All citations have been corrected as indicated in the plans of correction.
