Comments:

An unannounced renewal inspection was completed by 2 Licensing Inspectors from 11:45 a.m.-1:30 p.m.on 9/27/2019 and from 5:00 p.m.-6:00 p.m on 9/30/2019 The facility has 4 residents in care at this time. There were 4 residents on the premises during the inspections and 2 staff members were on duty. Administrator was present during the inspection. Lunch meal was observed. Residents interviewed did not voice any concerns or complaints. A sample of 3 resident's records and 2 staff records were previously reviewed for compliance. New personnel record reviewed since last inspection for criminal history reports. See the violation notice for noncompliance. A medication administration pass, physician's orders and medications were observed. A tour of the physical plant was completed. First aid kit supplies were observed. Description:	Based on a review of staff record, all required information was not available for review. Evidence: Staff member #1 criminal background check was not available.
Plan of Correction:	Administrator will make sure that the background checks are done and completed upon hiring and will keep a copy of the background check and receipt of payment in staff's folder until the original copy is returned to the facility.
