Comments:
The licensing inspector for Greendale Home conducted a one day non mandated complaint investigation at Greendale Home on 09/18/2019 regarding a compliant that had been received by the licensing office. The Complaint reported there was an issue with the cleanliness of the building, cleaning supplies available, and resident care. The inspector found a preponderance of evidence to support some of the allegations within the report. Therefore the complaint was found to be valid. An exit meeting was held with the administrator and at that time an opportunity was given to find items not available in resident files. As a result of this inspection two violations are being cited. Please develop a plan of correction for each cited violation along with a date of correction and return a signed copy back to the licensing office within 10 days (10/04/2019) of receipt. Description: Based on interviews with staff and documentation review, the facility failed to report to the regional licensing office with 24 hours any major incident that has negatively affected or that threatens the life, health, safety, or welfare of any resident.

EVIDENCE:
1. Resident # 1 was admitted to the facility on September 4, 2019. According to documentation review and an interview with the administrator on the evening of September 12, 2019 this resident became aggressive and argumentative with staff and other residents. An Emergency Custody order was granted on the morning of September 13, 2019 resulting in this resident being escorted to the hospital by law enforcement. Illegal drugs were also discovered in this resident's room by staff. He was admitted to the hospital on September 13, 2019. This incident was not reported to the licensing inspector within 24 hours.

Plan of Correction: The facility will report to the regional licensing office within 24 hours any major incident that has negatively affected or that threatens the life, safety, or welfare of any resident. DON/Supervisor will fax all incident forms to regional licensing office within 24 hour after the incident, Administrator will monitor for compliance. [sic]

Description: Based on documentation review and interviews with staff, the facility failed to ensure every resident was treated with respect courtesy and consideration as a person of worth, sensitivity and dignity.

EVIDENCE:
1. Resident # 1 was admitted to the facility on September 4, 2019. This resident became agitated, aggressive and argumentative with staff on the evening of September 12, 2019. According to interviews with staff an argument began between Resident # 1 and staff # 2 in the downstairs area of the facility. Both Resident # 1 and Staff # 2 came upstairs to the Administrators office still arguing. Staff # 3 witnessed staff # 2 arguing with the resident and calling him crazy. This resulted in staff # 2 to receive a written warning not engage in arguments or call residents names under any circumstances.

Plan of Correction: All residents will be treated with courtesy, respect, and consideration as a person of worth sensitivity, and dignity. All staff will attend a mandatory in service on resident rights, and behavior management the person centered way, adults with mental impairment. [sic]
