An unannounced inspection was conducted on 9/25/2019 at the facility to investigate a complaint regarding community support services being provided to seven residents. Licensing staff interviewed facility residents and the administrator, and reviewed facility/file documentation to determine compliance. Based on the interviews and file documentation, the complaint was not supported and was determined to be not valid.
