Description:	termined the facility did not ensure a special diet, was prepared and served according to the physician's or other prescriber's orders. Evidence: The posted menu is a la carte. This allows residents to choose whatever meals they would like. A diet board was not up to date with Resident A special diet. The Chef was unable to specify which desserts were concentrated sweet. The menu was not dated, and snacks were not specified. The facility is not maintaining a listing of the menu that is served for residents on a special diet, and residents with serious cognitive deficit.
Plan of Correction:	Nursing staff or designee enter diets into the EHR (electronic health record) and prints diet report to post in kitchen area. This report is updated as needed with changes by nursing team or designee. The Executive Chef has updated the menu to specify NCS deserts, added the date, and what specific snacks are available. The Executive Director will complete monthly checks in the dietary department to verify that the modified diet report is correct and posted and that the menus reflects NCS deserts, date, and snacks available.
