Comments:

Two licensing inspectors conducted a joint investigation with Adult Protective Services from the local department of social services regarding a complaint that had been received by the licensing office on 10/15/2019 regarding resident care. The complaint investigation took place at Greendale Home on 10/22/2019. The investigation started at 10:49 am and concluded at 11:15 am. Allegations regarding resident care were investigated including bruising found on the resident. Resident and staff interviews were completed, the resident was observed, and documentation was gathered. There was not preponderance of evidence to support the allegations within the complaint. Therefore, the complaint is determined to be not valid. An exit meeting was held with the Administrator and other key staff on 10/22/2019. At that time an opportunity was given to find items not available in files. The results of this investigation were reviewed and as result of this inspection no violations are being cited.
