Comments:
Two licensing inspectors conducted an investigation at Dogwood Crossing on 10/25/2019 regarding a complaint that was received by the licensing office including allegations about resident care . The inspection started at 10:25 am and concluded at 10:50 am. During the inspection staff interviews were conducted, documentation and the resident record was reviewed, and additional information was gathered. Based on interviews and documentation there was not a preponderance of evidence to support the allegations in this complaint; therefore, the complaint is not valid. One violation is being cited as a result of this complaint investigation. An exit meeting was held with the administrator and other key staff on 10/25/2019 and at that time an opportunity was given to find items that were not available in files. Please develop a plan of correction for the violation cited along with a date to be corrected and return to the licensing office within 10 calendar days (11/07/2019) of receipt. Description: Based on resident record review and documentation review, the facility failed to include a resident's inability to use the call signaling device on the Individualized Service Plan (ISP).

EVIDENCE:
1. Resident # 1 was admitted to the facility on 10/04/2019.
2. On 10/06/2019 this resident was found crawling in the floor on his hand and knees at approximately 9:00 am. He had knocked over the night stand, breakfast plate and had removed all of his clothing. Upon further observation it was discovered this resident had carpet burns on his knees. This resident had a call signaling device around his neck at the time of his fall that he did not activate.
3. His Uniform Assessment Instrument (UAI) dated 10/04/2019 listed this resident is disoriented in all spheres some of the time. His physical dated 10/04/2019 lists a diagnosis of Lewy body dementia and he is prescribed Aricept 10 mg tablets one at bedtime for dementia. This resident is also diagnosed with multiple falls.

Plan of Correction: Mater Preliminary plan of care, ISP was updated to include call bell device. Usage and understanding. [sic]
