Comments:

Two licensing inspectors conducted an unannounced one day inspection of a self reported incident at Commonwealth Senior Living in Cedar Bluff on 10/31/2019. The inspection began at 11:00 am and concluded at 11:32 am. The licensing inspectors observed the interactions between staff and residents and reviewed staff files. An exit meeting was held with the administrator of the facility on 10/31/2019. Two violations are being cited as a result of this inspection. Please provide a plan of correction; date to be corrected for each violation cited and return a signed and dated copy of the violation notice back to your licensing inspector within 10 days (11/11/2019) of receipt. Violations:

Standard #:	22VAC40-73-110-1
Description:	Based on observation of video footage and staff interviews the facility failed to ensure that all staff was considerate and respectful of the rights, dignity, and sensitivities of persons who are aged, infirm, or disabled. EVIDENCE: 1. Staff #1 did not exhibit behaviors to Resident # 1 that displayed consideration, respectfulness, and sensitivity. 2. During video footage observed by two licensing inspectors, Staff #1 was observed swatting at Resident #1's hands during the night shift of 10/21/2019. 3. Staff # 2 stated when she interviewed Staff #1 over the phone, Staff #1 said she did nothing wrong. She stated that she does hit at Resident #1 but would never hurt the and Resident #1 bothers a lot of the staff members.
Plan of Correction:	Staff member #1 was suspended on 10/24/19 and terminated on 10/28/19. [sic]

Description:	Based on information gathered during video footage and staff interviews, the facility failed to uphold the rights of one resident by failing to ensure one resident was free from threats or other degrading or demeaning acts against her. EVIDENCE: 1. Staff # 3 stated she heard Staff #1 make a verbal threat to slap another resident.
Plan of Correction:	Staff member #1 suspended on 10/24/19. Staff member #1 was terminated on 10/28/19. All staff in-serviced on resident rights/abuse, and mandated reporting was conducted. [sic]
