Comments:
The Piedmont Licensing Office received an anonymous complaint in regards to The Elms of Lynchburg. The complaint alleges that the facility air conditioning is not working, the building is hot and that the doors to the safe secure unit are being left open with resident roaming at will.

The LI for The Elms of Lynchburg conducted an unannounced complaint investigation at the facility on 7/18/19 from 2:30pm until 4:00pm. The facility physical plant was observed and interviews were conducted with residents and staff. Based on the preponderance of all information available for review this complaint is founded. Please respond back with a plan of correction within 10 days of receipt of this notice. If you have any questions please feel free to contact your LI at 540-309-2968.

Description: Based on physical plant observations and staff interviews, the facility failed to ensure that temperatures in all areas used by residents did not exceed 80?F.

EVIDENCE:

1. At 2:10pm on the day of inspection, the LI in conjunction with the facility Administrator noted that the temperature in the hallway of the A-Hall was at 82'F. The following rooms on the A Hall were noted to have residents residing in the rooms at 2:30pm with temperatures above 80'F; Room 103 with a temperature of 85'F, Room 106 with a temperature of 82'F, Room 108 with a temperature of 83'F and Room 112 with a temperature of 84'F. The Dining Room which is used by resident from the A-Hall and B-Hall was noted to be at 81'F at 2:45pm.

2. At 3:15pm on the day of inspection, the LI in conjunction with the facility Administrator noted that the temperature in the hallway of the Safe Secure Unit was at 82'F. The following rooms on the Safe Secure Unit were noted to have residents residing in the rooms at 3:20pm with temperatures above 80'F; Rooms 309 and 310, both with a temperature of 85'F.

Plan of Correction: All temperatures in ALF and Memory Care have been maintained below 80 degrees. Portable a/c units were temporarily placed to ensure the temperatures met the standards for our residents. A local HVAC Company was sent to repair units that were not operating appropriately. Currently, a new a/c unit has been installed and operating properly.

Description: Based on observations and staff interviews, the facility failed to develop and implement a plan to protect residents from heat-related and cold-related illnesses in the event of loss of air-conditioning or heat due to emergency situations or malfunctioning or broken equipment.

EVIDENCE:

1. The LI requested to review the facility plan to protect residents from heat-related and cold-related illnesses in the event of loss of air-conditioning or heat due to emergency situations or malfunctioning or broken equipment. Staff were unable to locate this plan for the LI to review during this inspection.

Plan of Correction: A Heating and Cooling resident emergency plan has been developed and implemented to ensure staff is aware of procedures to follow to ensure our residents are safe during heat-related or cold-related emergencies.
