Comments:
An unannounced inspection was conducted at the facility on 11/22/2019 to followup on a self reported incident regarding facility accessibility and accommodations. Based on interviews with one resident and facility staff, a review of the license issued and a review of the Certificate of Occupancy, one violation was cited. Please complete the "plan of correction" and "date to be corrected" for the violation cited on the violation notice and return to the Inspector within 10 days. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the non-compliance with the standard, 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Violations:
Standard #: 22VAC40-73-40-A
Description: Based on a inspection at the facility on 11/21/2019, the licensee failed to comply with the terms of the license in regards to the type of residents permitted.

Evidence: The license issued specifies "All residents must be capable of responding to an emergency situation without physical assistance from staff". Resident # 1 is a double amputee who relies on an electric wheelchair for mobility. The facility does have a small paved walkway that leads to a gravel driveway and a first floor entrance that does not require a ramp. However, the driveway has thick gravel which makes it difficult for the resident to maneuver his wheelchair. The resident became stuck in the driveway recently and required the physical assistance of a local police officer to become unstuck. The resident reported during an interview on 11/22/2019 that he "feels a prisoner" as he is restricted to the small walkway and requires physical assistance to maneuver the driveway.

Plan of Correction: Residents will be assisted with relocation to a facility that can better meet their accessibility needs.
