An unannounced monitoring study was conducted on 11/19/2019 in response to the facility's probationary status. At the time of entrance 81 residents were in care. The sample size consisted of nine resident records, one staff record, and two individual interviews. Resident and staff records and other documentation were reviewed. Residents were observed engaging in activities including chair exercises and word in word (crossword puzzle). Medication administration was observed. No violations were cited and an exit interview was held.
