Comments:

The LI for Town Creek Assisted Living Facility conducted an unannounced complaint inspection on 6/25/2019 from 9:30am to 2:00pm under the supervision of the LA. Resident and staff records as well as other forms of facility documentation were reviewed and interviews were conducted with residents and staff. Based on all information available for review, the facility followed physicians orders for resident 1's prescribed medications and scheduled lab visits. An interview with resident 1 expressed that he had cut down on smoking and was using smokeless tobacco products more. Resident 1 also expressed that he was happy at the facility, that staff were very helpful and that he aware of his rights to be able to smoke or dip as he chose to do so.
