Comments:

An unannounced monitoring inspection was conducted on 12-05-19 (ar 09:05 am/ dep 17:25 p.m). The facility census was 89. The focus of the inspection was on risk rating of B-2 or above from the renewal inspection. A review of sampled residents from all three buildings was conducted. The entrance and exit interview was conducted with the administrator. The exit was also attended by the executive director. The acknowledgement form was signed and dated by the administrator. Comment: LI suggested facility develop a method of noted correction to previous violations or documentation of information noted during facility's internal audit to avoid violations or being cited again for the same violation; use of sticky notes, writing correction on the violation document or having a tracking sheet per record, etc.
