An announced initial inspection was conducted on 12-11-19 (ar 10:43 am/dep 12:43pm).The facility census was 40. The administrator shared with the inspector the changes to the rooms and the number of rooms that were removed for conversion on the assisted living and safe, secure unit. The maximum number of capacity with the current census will be 41, a change from the previous 48. A tour of the facility was conducted with the administrator and maintenance director. There were no violations noted during this initial inspection.The administrator was present during the exit and signed the acknowledgement form.
