Comments:

An unannounced monitoring inspection was conducted on 12/3/19 by 1 licensing staff from approximately 8:00 a.m. - 11:15 a.m. Previous violations were reviewed and corrected. There are currently 13 residents in care at the facility. During the inspection, a tour of the facility was conducted, a sample of 6 resident and 3 staff records were reviewed, facility documentation was reviewed, facility postings were reviewed, first aid kit and emergency food/water supplies were checked, medication pass was observed and medication administration records and physician orders were reviewed. An exit meeting was held with the administrator. Violations were cited during this inspection. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to the Inspector within 10 days. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #:	22VAC40-73-260-A
Description:	Based on review of staff records and facility documentation, the facility failed to ensure that each direct care staff member maintain current certification in first aid. Evidence: Staff #1 (date of hire: 4/4/12) first aid certification expired on 3/27/19.
Plan of Correction:	Administrator will ensure that staff members first aid remains current by giving monthly notice.
Standard #:	22VAC40-73-320-B
Description:	Based on review of resident records and facility documentation, the facility failed to ensure that a risk assessment for tuberculosis (TB) be completed annually on each resident as evidenced by the completion of the current screening form published by the Virginia Department of Health or a form consistent with it. Evidence: Resident #2 (date of admission: 11/1/18) last completed TB risk assessment was dated 10/26/18.
Plan of Correction:	Administrator will ensure that all risk assessments for TB be completed annually by giving physician a month notice to maintain current.
