Technical Assistance:
T/A - Orientation of all residents on use of the signaling system was documented. However, resident (#2) was unable to express or demonstrate how to use the system when interviewed by licensing staff and local social services It was recommended that the facility re- oriented all residents on use of the signaling system.

Comments:
An unannounced inspection was conducted at the facility with local social services to investigate a complaint of a resident to resident altercation.One of the allegations was determined to be valid. Violations were cited as a result of the inspection.Please complete the "plan of correction" and "date to be corrected" for the violations cited on the violation notice and return to the Inspector within 10 days. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the non-compliance with the standard, 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). See violation notice for non-compliance.

Description: Based on a report from local adult protective services and an interview with the administrator of the facility, the facility failed to report a major incident that negatively affected or threatened the health, safety or welfare of a resident in care to the regional licensing office.

Evidence: On 11/11/2019 a resident was seen at a local hospital with a busted lip, scratches across her face and bite marks on her body. The resident was admitted to the hospital. The incident was reported to the licensing office on 1/14/2019 by local social services. The facility did not report the incident.

Plan of Correction: All physical altercations between residents will be documented and report sent via fax to licensing specialist.

Description: Based on an interview with the facility's administrator, medical attention from a licensed health care professional was not secured immediately for injuries suffered by a resident.

Evidence: Resident # 1 received injuries at the facility during an altercation with another resident on 11/11/2019. Resident # 1 did not receive medical attention by a licensed health care professional for the injuries until the following day when she was transported to the local ER (emergency room) from a day program.

Plan of Correction: Staff will document when a resident is asked to go to the hospital and if they refuse have them sign they refused - if any injuries need first aid care. A report will be faxed to the licensing specialist.
