Comments:

An unannounced complaint inspection was conducted on 12-5-19 (ar 09:05 am/dep 17:25 pm) and 12-10-19 (ar 08:20 am/ dep 10:30 am) in response to complaint that came to the department on 11-27-19. Record was reviewed and interviews conducted regarding allegations of resident not receiving medications, discrepancy with billing, resident medical appointments and resident care. The information gathered did not support the allegations, therefore the violation is determined not valid. There was a violation and technical assistance provided regarding a non complaint issue. Comment: The posted menu should be in at least 12 font. Please complete the columns for "description of action to be taken" and "date to be corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 calendars of receipt.

Description:	Based on observation and staff interview, the facility failed to ensure the breakfast menu for the week was dated and posted in an area conspicuous to residents. Evidence: 1. On 12-10-19 during a review of the posted menu it was observed that the breakfast menu was not posted for the residents. 2. Staff #1 was informed and acknowledged the breakfast menu for the day/week was not posted for the residents.
Plan of Correction:	1. The breakfast menu for the day and week was posted by the Assisted Living Director at the time of the inspection. 2. Re-education will be conducted by the Executive Director / Designee to the Assisted Living Directors no later than January 4th, 2020. 3. The meal menus will be monitored daily for four (4) weeks to verify posting and compliance. 4. To monitor for ongoing compliance, the menu postings will be verified by the Assisted Living Director / Designee weekly and associates assigned to the dining room will monitor for postings daily.
