Comments:

Two licensing inspectors conducted an investigation at Dogwood Crossing on 01/14/2019 regarding a complaint that was received by the licensing office including allegations about resident care, the physical plant, and issues with confidentiality. The inspection started at 12:15 pm and concluded at 1:09 pm. During the inspection staff interviews were conducted, documentation review and residents interviews were completed. The physical plant was observed. Based on interviews, documentation review and the observance of the physical plant there was not a preponderance of evidence to support the allegations in this complaint; therefore, the complaint is not valid. An exit meeting was held with the administrator and other key staff on 01/15/2020 and at that time an opportunity was given to find items not available during the inspection. As a result of this inspection no violations are being cited. Please return a signed and dated copy of this summary report back to the licensing office.
