Comments:

Two licensing inspectors along with the local department of social services and law enforcement conducted a complaint investigation regarding a complaint that was received by the licensing office on 01/14/2020. Allegations included issues with resident care and financial misconduct.The complaint investigation was conducted at Brookdale Bristol on 01/16/2020. The inspection started at 1:30 pm and concluded at 3:11 pm. During the inspection interview with staff were completed. The resident record was reviewed along with physicians notes, fall risk ratings and incident reports. A detailed list of the resident's financial statements were reviewed. There was not preponderance of evidence to support the allegations in this complaint; therefore, the complaint is not valid. An exit meeting was held with the administrator and other key staff on 01/16/2020 and at that time an opportunity was given to find items not available during the inspection. As a result of this inspection no violations are being cited. Please return a signed and dated copy of this summary report back to the licensing office.
