Comments:
Two licensing inspectors conducted an unannounced non-mandated focused monitoring inspection at Carrington Place in Wytheville on 02/03/2020. The inspection started at 10:45 am and concluded at 11:20 am.The purpose of this inspection was to look at previous violations and determine compliance with standards using the most recent inspection protocol. During the inspection the building was observed, resident and staff files were reviewed, medication carts were observed, physicians orders and medication administration records were reviewed. An exit meeting was held with the director of nursing and other key staff on 02/03/2020 and at that time opportunity was given to find items not available during the inspection. As a result of this inspection one violation is being cited. Please develop a plan of correction for the violation cited along with a date of correction and return a signed and dated copy back to the licensing office within 10 calendar days (02/13/2020) of receipt. Violations:
Standard #: 22VAC40-73-660-B
Description: Based on documentation review and a tour of the physical plant, the facility failed to ensure that residents maintaining medications in their rooms were rated independent in medication administration on their Uniform Assessment Instrument (UAI).

EVIDENCE:
1. Resident # 8 is rated dependent in medication administration according to his Uniform Assessment Instrument.
2. Resident # 8 is prescribed Proventil AER HFA inhaler 1-2 puffs by mouth every four hours as needed for COPD and is available for this resident.
3. Staff # 1 stated he keeps this medication bedside and self-administers.
4. The physician's order does not state that Resident # 8 may self-administer this medication.

Plan of Correction: Staff obtained an order for Resident #8 (may keep at bedside) and self-administer inhaler on 02/03/2020. Quality review of current residents that self-administer medications also have a Physicians order (may keep at bedside) by RCDNS/Designee by (02/10/2020). Follow up based on findings. RMA's re-educated by RCDNS/Designee, that residents that self-administer medications must have a Physician order stating they may keep medication at bedside. RCDNS/Designee will monitor weekly times 12 weeks. [sic]
