Comments:

An unannounced inspection was made at the facility to follow-up on a facility self reported incident. A second report was received subsequently by local adult protective services. A violation was cited as a result of the incident. Please complete the "plan of correction" and "date to be corrected" for the violation cited on the violation notice and return to the Inspector within 10 days. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the non-compliance with the standard, 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). See violation notice for non-compliance. Licensing staff requested and reviewed the facility's policy and procedures for fall prevention The provider's responses for the "plan of correction" were not received as of February 3, 2020 and will not appear on this "Violation Notice."

Description:	Based on two reports and interviews, facility staff failed to be considerate and respectful of the rights, dignity, and sensitivities of persons who are aged, infirm, or disabled. Evidence: Reports from the facility and local adult protective services were received that staff # 1 hit resident # 1 with a "shoe horn". The resident was reported to be laying in bed and would not get up. The resident sustained red marks on both hands from the shoe horn. Residents of assisted living facilities have rights as provided in ? 63.2-1808 of the Code of Virginia: (1) to be treated with courtesy, respect and consideration as a person of worth, sensitivity and dignity and (2) to be free from physical abuse, threats and other degrading or demeaning acts.
