Comments:

Two licensing inspectors conducted a one day initial licensing inspection at Gray Ridge Village on 01/09/2020. The inspection started at 1:45 pm and concluded at 2:30 pm. During the inspection the building, resident rooms, the first aid kit, and emergency food and water supplies were observed. An exit meeting was held with the administrator and other key staff on 01/09/2020. At that time findings of the inspection were reviewed. As a result of this inspection one violation is being cited. Please develop a plan of correction for the violation cited along with a date of correction and return a signed and dated copy back to the licensing office within 10 calendar days (02/14/2020) of receipt. Description:	Based on observations made during the tour of the building, the facility failed to have the name and telephone number of the appropriate regional licensing supervisor of the department. EVIDENCE: 1. The Licensing Inspector observed the rights and responsibilities of residents posted on the wall with the previous regional licensing supervisor and telephone number instead of the current licensing administrator supervisor and telephone number.
Plan of Correction:	Current name and telephone number of appropriate regional licensing supervisor of the department will be posted. Admin to monitor. RCD to monitor. Corrected on the day of inspection.
