Comments:
The licensing office received information in a facility self-report from English Meadows Blacksburg on February 14, 2020 regarding a medication error. A monitoring self-report inspection has been conducted. As a result of the information received by the licencing office 1 violation is being cited. Please develop a plan of correction for the cited violation along with a date of correction and return a signed and dated copy back to the licensing office within 10 calendar days (02/29/2020) of receipt. if you have any questions or concerns please contact your inspector at 276-608-3514. Violations:
Standard #: 22VAC40-73-680-D
Description: Based on information sent to the licensing office as a facility self-report, the facility failed to administer all medications in accordance with physician's instructions and consistent with standards of practice outlined in the current registered medication aide curriculum approved by the Virginia Board of Nursing.

EVIDENCE:
1. Resident # 1 was admitted to the facility on 09/25/2017. She is prescribed Citalopram HBR 10 mg tablet, take one tablet by mouth once daily for anxiety, Cutlurelle Digestive Health Lactobacillus caps, take one capsule by mouth everyday for bowel health, Haloperidol 2mg/ml solution, take 0.5 ml (1mg) by mouth three times daily for dementia with behaviors, and hydrocodone/APAP 5-325 mg tablet, take one tablet by mouth twice daily for pain.
2. On the morning of February 14, 2020 resident # 1 was administered the above medications at approximately 7:15 am by staff # 1. According to information sent to the licensing office by the facility staff # 1 forgot to sign on the EMAR that he had administered these medications to resident # 1.
3. Staff # 2 relieved staff # 1 and administered the same above medications to resident # 1 approximately 40 minutes later as the EMAR had not been signed off on.
4. Hospice and the facility staff conducted 30 minute checks and vital signs on resident # 1 for 24 hours to monitor any adverse effects.

Plan of Correction: 1. Immediate corrective action and education was performed with RMA.
2. RMA involved is no longer employed at community.
3. Resident was monitored by staff and hospice nurse per MD orders.
4. All Med Staff received education regarding proper medication administration practices. [sic]
