Comments:

An unannounced monitoring inspection was conducted on 2/26/2020. At the time of entrance six residents were in care with three staff providing care. The sample size consisted of three resident records and two staff records. Resident and staff records and other documentation were reviewed. Background checks of all staff hired since the previous inspection on 10/23/18 were reviewed. Residents were observed eating breakfast and and engaging in activities including music appreciation and current events. Medication administration was reviewed. Violations cited and assigned risk ratings were reviewed during the exit interview. Description:	Facility failed to ensure that the interior and exterior of all buildings shall be maintained in good repair and kept clean and free of rubbish. Evidence: The backsplash over the kitchen sink is dirty and stained, fan enclosure above the stove is discolored, a knob is missing for one of the burners and the cabinets are scratched and the finishing is wearing off on them.
Plan of Correction:	Owner will have the kitchen deep cleaned and will have the cabinets painted. Stove will be repaired or replaced.
