Comments:
An unannounced renewal inspection was completed on March 05, 2020, from 2:00 p.m. to 4:04 p.m. for Metro Gardens Assisted Living Facility. Upon arrival the licensing representative met with the administrator in order to initiate the entrance conference. The administration confirmed the census as fourteen (14) residents in care. After the entrance conference, the administrator and licensing representative toured the building for compliance. The record review encompassed eight (8) resident records and four (4) staff records. Residents were observed in active daily activities. All prior cited violations were corrected but new violations were cited.

Please submit a written plan of correction within the next ten calendar days. Your plan should specify how you will correct each violation , an intended plan for the future compliance, a date of correction for each violations, and the job title of the staff member responsible for implementing preventive measures. Standard #: 22VAC40-73-300-B
Description: VIOLATION: Based upon observation and an interview with the administrator, the facility failed to utilize the facility's written communication logs in order to inform each direct care staff of significant incidents concerning each residents in care.

EVIDENCE: During the renewal inspection, the facility failed to update the communication logs regarding an authorization for treatment for a resident who was injured while attending day support. The administrator was notified of the residents treatment when the hospital called the facility to discharge the resident. The administrator confirmed that the on duty morning staff member failed to update the communication logs regarding the residents recent injury and treatment.

Description: VIOLATION: Based upon observation, the facility failed to maintain certain areas of the building.

EVIDENCE#1: The flooring throughout the building was stained and slightly discolored.

EVIDENCE #2: One of the second floor ceiling tiles,near the bathroom entrance, was protruding from the wall which left a small open space in the ceiling.

Description: VIOLATION: Based upon the building tour, the administration team failed to ensure that the facility was well ventilated and free from odors.

EVIDENCE: During the building tour the licensing representative noticed that the facility had a strong mildew and musty odor.
